If you're a MA resident and have given us a mailing address outside of the Commonwealth, please contact us at 1099HC @ ConsolidatedHealthPlan.com to receive your 1099HC form.

Massachusetts Health Care Notice

The State of Massachusetts requires all residents to be insured. Proof of insurance coverage will be requested on your State Tax Return.

Consolidated Health Plans will send a Health Care 1099HC statement to each insured at his/her address on file.  If the address we have on file is not a Massachusetts address, you will not receive a 1099hc.   Therefore if you are a Massachusetts resident with an out of state address, you need to send us your in-state address.

Do not throw this form out. You will need it to complete your 2007 Massachusetts Tax Return.
